“The Master Assessment” is the most comprehensive lower back assessment you will ever take. It normally takes 3-4 hours to analyse a patient with the sole purpose of finding the root cause of the lower back pain.

Detailed History

One of the oldest and most valuable forms of gathering information in medicine to reach a successful diagnosis has always been listening to patients tell their full story. Unfortunately, in modern medicine, this vital exchange is being lost due to time constraints and relying on imaging studies. On the contrary, The Master Assessment in HumanRx relies more than 60% of its merits on the past history that you, as my patient, bring along. No stone will remain unturned as we take into consideration everything from your childhood accidents to everyday leisure activities.

Current Pain Triggers

Often, patients think their back pain started when they picked up a paper from the floor or lifted a golf bag to the car. In reality, this was only a trigger event, but the actual cause usually lies deeper. Due to how our medical system has been built, most orthopaedic surgeons and physical therapists have no time to “look deep enough” to understand all the important factors.
To find the specific cause of your lower back pain, I want to understand if your pain is triggered by prolonged sitting, standing up from sitting, standing for too long, bending forward, putting on your shoes, or sleeping. These are only a few possible pain triggers we will go through as part of the assessment.

Sleep

Have you ever considered why your back pain is more likely to flare up during stressful periods in your life? How was your sleep during these times? Less hours? More awakening? Less rested in the morning?
A healthy sleeping schedule is directly related to back pain. It is during sleep that micro injuries from daily activities have a chance to heal. Furthermore, a bad sleeping routine acts as a domino effect for the human brain and neuro-muscular system alike.

Nutrition

Have you ever heard about anti-inflammatory and pro-inflammatory foods? Put simply, anti-inflammatory foods will aid the healing process to build and repair your muscles, bones, joints, and nervous system. Whereas, pro-inflammatory foods will increase the hypersensitivity of your nervous system, and thus, increase lower back pain.

Neuro-Orthopaedic examination

The true examination begins the moment you step into HumanRx. I will be observing how you climb the stairs, take off your shoes, hang up your coat, and sit down. These movements are all part of your daily habits, which might have major contributions to your injury.
During the highly specialized neuro-orthopaedic manual testing, I will use various scientifically proven tests developed by leading experts in the field, including Dr. Stuart McGill, Dr. Pavel Kolar, and Michael Shacklock. This allows me to understand the true cause of the spinal injury and create a customized rehabilitation program for each patient’s individual spinal injury.

Management strategies

During the final part of the examination, I will give a detailed explanation of the findings and suggest a suitable course of action. Once it has been established that the cause of your lower back pain is not a medical emergency and its origin is a mechanical injury, I will offer you multiple options regarding how and with whom to treat it.
If it is revealed, for example, that your pain is likely from an inflammatory condition such as ankylosing spondylitis or mechanical in origin but emergency such as cauda equina syndrome, I will immediately refer you to the best specialists.
If you decide to continue your treatment with me,  and depending on the acuteness of the injury, the rehabilitation process generally consists of 3 stages:

Stage 1: Survival phase. The time when pain is the most prominent and, therefore, impedes the patient from fully performing daily activities. Often, work, family, and social life are strongly affected.
During this phase, different manual therapy and specialized exercises will be used to speed the healing process.
By the end of this phase, you will have far less pain, and you will have regained some basic functionality such as walking longer distances or being able to sit longer.
Typically this stage takes between 3 to 6 months.

Stage 2: Rehabilitation phase. Once the acuteness of the pain starts to subside, you will become more functional and start to feel real enhancements in your daily performance and quality of life.
The main drivers in this phase are specialized exercises, which keep reinforcing new movement patterns but with added weight. By the end of this phase, you will be prepared to withstand usual everyday stresses. Typically, this phase takes between 6 to 12 months.

Stage 3: Performance phase. This phase is meant for people who want to achieve a physical goal that exceeds their earlier physical capabilities.
Do you dream about participating in a marathon? Or an Ironman competition? Or climbing a mountain? No problem, I can help you achieve these goals.
Most of my back pain patients don’t need this phase because they are happy to live a back-pain-free life. On the other hand, if you wish to return to sporting activities, the Performance phase is recommended since it prepares your body and spine for increased stress. The length of this phase varies depending on your athletic background and goals.
